ABSTRACT

The conventional approach to investigating functional connectivity in the block-designed study usually concatenates task blocks or employs residuals of task activation. While providing many insights into brain functions, the block design adds more manipulation in functional network analysis that may reduce the purity of the blood oxygenation level-dependent signal. Recent studies utilized one single long run for task trials of the same condition, the so-called continuous design, to investigate functional connectivity based on task functional magnetic resonance imaging. Continuous brain activities associated with the single-task condition can be directly utilized for task-related functional connectivity assessment, which has been examined for working memory, sensory, motor, and semantic task experiments in previous research. But it remains unclear how the block and continuous design influence the assessment of task-related functional connectivity networks. This study aimed to disentangle the separable effects of block/continuous design and working memory load on task-related functional connectivity networks, by using repeated-measures analysis of variance. Across 50 young healthy adults, behavioral results of accuracy and reaction time showed a significant main effect of design as well as interaction between design and load. Imaging results revealed that the cingulo-opercular, fronto-parietal, and default model networks were associated with not only task activation, but significant main effects of design and load as well as their interaction on intra- and inter-network functional connectivity and global network topology. Moreover, a significant behavior-brain association was identified for the continuous design. This work has extended the evidence that continuous design can be used to study task-related functional connectivity and subtle brain-behavioral relationships.

ABSTRACT

To systematically evaluate the clinical efficacy and safety of Xinmailong Injection in the treatment of heart failure after acute myocardial infarction. Seven Chinese and English databases, namely CNKI, VIP, Wanfang, SinoMed and PubMed, EMbase, Cochrane Library, were retrieved from the establishment of the database to March 2020. Randomized controlled trials for Xinmailong Injection in the treatment of heart failure after acute myocardial infarction were screened out. Cochrane collaboration network bias risk assessment tool was used to evaluate the literature quality of the studies included, and RevMan 5.3 software was used for Meta-analysis. A total of 926 relevant literatures were retrieved, and 12 studies were finally included, involving 972 patients, including 486 patients in the treatment group and 486 patients in the control group. The quality of the literatures included was generally low. The results of Meta-analysis showed that Xinmailong Injection combined with Western medicine could decrease the levels of BNP(SMD=-5.90, 95%CI[-8.45,-3.36], P<0.000 01) and NT-proBNP(SMD=-2.28, 95%CI[-3.13,-1.43], P<0.000 01) and decrease the levels of cTnI(SMD=-2.91, 95%CI[-4.21,-1.60], P<0.000 1), increase LVEF(MD=4.67, 95%CI[4.19, 5.16], P<0.000 01), increased 6 MWT(MD=73.90, 95%CI[67.51, 80.28], P<0.000 01], decreased LVEDD(MD=-5.46, 95%CI[-9.66,-1.25], P=0.01), reduce the level of serum inflammatory factor(hs-CRP, CRP, IL-6). In terms of safety, less adverse reactions occurred in the study, with no impact on the treatment. The results showed that clinical use of Xinmailong Injection combined with Western medicine in the treatment of heart failure after acute myocardial infarction can further alleviate clinical symptoms and relevant indexes, with less adverse reactions. However, due to the limitations in quantity and quality of the clinical studies included, the positive results can only be used as a hint and reference for clinical diagnosis and treatment, and more high-quality studies are needed to further confirm its efficacy.

ABSTRACT

BACKGROUND@#Obesity is a fundamental factor in metabolic disorders such as hyperlipidemia, insulin resistance, fatty liver, and atherosclerosis. However, effective preventive measures are still lacking. This study aimed to investigate different surgical protocols for removing partial adipose tissue before the onset of obesity and determine whether, and by which protocol, preliminary adipose removal could exert potent preventive effects against diet-induced metabolic disorders.@*METHODS@#Male low-density lipoprotein receptor (LDL-R) knockout (KO) mice were randomly divided into four groups and subjected to epididymal fat removal (Epi-FR) surgery, subcutaneous fat removal (suQ-FR) surgery, both subcutaneous and epididymal fat removal (Epi + suQ-FR) surgery, or sham-operation. After 1 week of recovery, all mice were given a high-fat diet (HFD) for 10 weeks to induce metabolic disorders.@*RESULTS@#In the Epi-FR group and the sham-operated group, the mean numbers of the residual subcutaneous fat were 28.59 mg/g and 18.56 mg/g, respectively. The expression of relative genes such as Pparg, Cebpa, Dgat2, Fabp4 and Cd36 in the residual subcutaneous fat increased 2.62, 3.90, 3.11, 2.06, 1.78 times in the Epi-FR group compared with that in the sham-operated group. Whereas in the other fat-removal groups, the residual fat depots had no significant change in either size or gene expression, as compared with those of the sham-operated group. Plasma lipid and glucose levels and insulin sensitivity, as detected by the glucose tolerance test, were not significantly alleviated in the three fat removal groups. Liver mass or lipid content was not attenuated in any of the three fat removal groups. The atherosclerosis burdens in the entire inner aorta and aortic root did not decrease in any of the three fat removal groups.@*CONCLUSIONS@#Our data suggest that removal of epididymal adipose or subcutaneous adipose alone or in combination before the onset of obesity did not protect against hyperlipidemia, insulin resistance, fatty liver, or atherosclerosis in LDL-R KO mice fed with a HFD. Hence, adipose removal possibly does not represent a potential approach in preventing obesity-related metabolic disorders in the obesity-susceptible population.

ABSTRACT

Objective To summarize the integrative rehabilitation treatment of traditional Chinese medicine (TCM) and Western medicine in hypertrophic olivary degeneration (HOD) secondary to the operation of brain stem cavernous malformation. Methods The clinical data of medication, rehabilitation and follow-up of a patient with HOD secondary to operation on brain stem cavernous malformation was retrospectively analyzed. Results Three months after operation, limb static and motor tremor, dysarthria, palate spasm, eye movement disorder and walking difficulty appeared. The patient was diagnosed as HOD according to clinical features and brain magnetic resonance imaging (MRI). He was treated with pertinence rehabilitation training combined with TCM including acupuncture and herbs. After integrative rehabilitation, he could stand and walk independently, the tremor was alleviated, the balance function improved, the activities of daily living improved, and the dosage of oral western medicine also decreased. Conclusion After intracranial surgery, secondary neurodegeneration and movement disorder may appear, and it could be improved by integrative rehabilitation treatment of TCM and Western medicine

With the development of social economy and the transformation of modern medical models as the background, this paper expounds the importance of improving medical students' doctor-patient communication ability in terms of that doctor-patient communication can be helpful for doctors to deeply understand the illness, make accurate diagnosis, improve doctor-patient relationship, and reduce medical disputes. By analyzing the current situation and existing problems of doctor-patient communication teaching at home and abroad, the paper puts forward some suggestions on strengthening the training of medical students' doctor-patient communication ability from four aspects: cultivating professional communication skills, establishing multi-disciplinary teaching teams, adopting diversified teaching forms and formulating systematic evaluation system.

ABSTRACT

As traditional data management model cannot effectively manage the massive data in traditional Chinese medicine(TCM) due to the uncertainty of data object attributes as well as the diversity and abstraction of data representation, a management strategy for TCM data based on big data technology is proposed. Based on true characteristics of TCM data, this strategy could solve the problems of the uncertainty of data object attributes in TCM information and the non-uniformity of the data representation by using modeless properties of stored objects in big data technology. Hybrid indexing mode was also used to solve the conflicts brought by different storage modes in indexing process, with powerful capabilities in query processing of massive data through efficient parallel MapReduce process. The theoretical analysis provided the management framework and its key technology, while its performance was tested on Hadoop by using several common traditional Chinese medicines and prescriptions from practical TCM data source. Result showed that this strategy can effectively solve the storage problem of TCM information, with good performance in query efficiency, completeness and robustness.

ABSTRACT

Due to abstract and intricate theories of TCM theories, its essential characteristics are difficult to master, so the mechanism can be better explored through study on quantification of prescription properties. At present, quantification research by many scholars mainly adopts isolated research model limited to one or several kinds of TCM properties, which is difficult to reveal treatment mechanism of prescriptions. The development of big data and cloud computing technology provides a new way for the deep analysis of massive and complex TCM information. Based on syndrome differentiation, it is trending in prescription quantification to employ a comprehensive analysis by using "four properties - five flavors - channel tropism - ascending and descending, floating and sinking - toxicity -efficacy" overall on macroscopic level to analyze and quantify the characteristics of TCM prescriptions and their relationships among each other. This article reviewed recent research progress in the quantification of prescription properties and provided references for the modernization and specification of quantification of prescription properties.

ABSTRACT

Objective To evaluate the antioxidant activity of different concentrations of alcohol extracts from Paeonia Rockii pollen.Methods The antioxidant activity of different concentrations of alcohol extracts from Paeonia Rockii pollen was evaluated by cupric reducing power method, DPPH radical scavenging activity method and ABTS radical scavenging activity method.Results The amounts of antioxidant activity of gallic acid of anhydrous ethanol extract, 75% alcohol extract, 50% alcohol extract, 25% alcohol extract, and water extract from Paeonia Rockii pollen were 26.00, 28.33, 28.90, 14.98, and 9.24 mg/g, respectively. The sequence of the ability of scavenging DPPH free radical and ABTS radical was Vc > 50% alcohol extract > 75% alcohol extract > anhydrous ethanol extract > 25% alcohol extract > water extract.Conclusion The different concentrations of alcohol extracts from Paeonia Rockii pollen has relatively strong antioxidant activity, especially for 50% alcohol extract and 75% alcohol extract.

ABSTRACT

Despite the continuous progress and development of surgical and comprehensive treatment in recent years, the 5-year survival rate of patients with head and neck squamous cell carcinoma (HNSCC) has not been significantly improved.Therefore,improving treatment efficiency and therapeutic strategies are urgently needed.As one promising novel therapy,immunotherapy has been gradually applied in the treatment of multiple tumors including HNSCC with low toxicity and high specificity.In the immunotherapy of HNSCC,the researchers have further developed a combined systemic therapy from single cytokine therapy and achieved good effects. On the other hand, multiple cancer vaccine therapies including protein/polypeptide-dendritic cell vaccine have been put into clinical trials.In addition,immunotherapy against PD-1 and other immune checkpoints have received extensive attention,and relevant inhibitory antibodies have also been approved for the treatment of recurrent or metastatic HNSCC. Here, we briefly make a review about the progress of multiple immunotherapies for the treatment of HNSCC,including cytokine therapy,vaccine therapy and immune checkpoint therapy.

·AIM: To investigate the effect of femtosecond laser assistedcataractsurgeryconventional phacoemulsification surgery on corneal endothelial cell morphology and function. ·METHODS: This study included 80 cases of cataract patients (88 sicked eyes) treated in our hospital from January 2016 to January 2017. All patients underwent phacoemulsification combining with intraocular lens implantation for treatment. Based on the review of treatment, 40 cases ( 43 sicked eyes ) receiving femtosecond laser assisted cataract surgery were included into the observation group, 40 cases (45 sicked eyes) receiving conventional phacoemulsification only were divided into the control group. The proportion of patients whose postoperative uncorrected visual acuity >0.5 of the two groups were compared, and sac diameter, capsule size, intraoperative cumulative energy release (CDE) were recorded, the corneal endothelial cell density and variation were recorded before and after operation, the preoperative and postoperative ratio of hexagonal cells were compared between the two groups subsequently. and ·RESULTS: At 1d postoperatively, uncorrected visual acuity >0.5 of the observation group was no significant difference with that of the control group; the ratio was 74% at 6mo after surgery, which was significantly higher than that of the control group(53%),there was statistical significance (P < 0. 05). The diameter of the anterior capsulotomy,the size of the capsule,the grade Ⅱ nuclear CDE, and the grade Ⅲ nuclear CDE of the observation group were sharply different from those of the control group, there was statistical significance (P<0.05). The differences of corneal endothelial cell density and variation, proportion of hexagonal cells before and after surgery of the two groups were significant, and the difference after 6mo between the two groups was huge, there was statistical meaning (P<0.05). · CONCLUSION: Femtosecond laser assisted cataract surgery can reduce the effect of ultrasound energy on cornea,and can sharply decrease the damage to corneal endothelial cell morphology and function.

ABSTRACT

·AIM: To evaluate the effect of femtosecond laser assisted cataract surgery combined with triple - focus intraocular lens implantation in the treatment of cataract. ·METHODS:Totally 86 cases (106 eyes) of patients with double cataract in our hospital from January 2016 to January 2017 were selected, including 49 cases (59 eyes) were set as the observation group ( treated with femtosecond laser assisted phacoemulsification combined with triple-focus intraocular lens implantation), and 37 cases (47 eyes) were set as the control group (received traditional phacoemulsification combined with triple -focus intraocular lens implantation). Corneal endothelial cell density,cumulative dissipated energy(CDE),distant and near visual accommodation before and after operation were compared between the two groups, postoperative complications were observed. ·RESULTS: The preoperative corneal endothelial cell density of two groups had no significant difference (P>0.05). The corneal endothelial cell density of two groups significantly decreased at postoperative 1wk,with statistic significance within groups (P<0. 05), and the corneal endothelial cell density of the control group was significantly lower than that of the observation group at postoperative 1wk (P<0.05). The CDE of the observation group were lower than that of the control group, and the difference between the two groups was significant (P<0.05). There were significant differences in best corrected visual acuity at different time points between the two groups (P<0.01). The best corrected visual acuity at 1 and 3mo after operation in the observation group was higher than that in the control group, and the difference was significant (P<0.05). There was no capsular tear in the observation group,while the incidence rate was 4.3% in the control group, with no significant difference between the two groups(P>0.05). The incidence of glare and halo in the observation group was 10.2% and 8.5% in the control group,and are in the patients whose age was above 60 years old, there was no significant difference between the two groups (P>0.05). · CONCLUSION: Femtosecond laser assisted cataract surgery combined with triple - focus intraocular lens implantation in the treatment of cataract can not only improve curative effect, but also provide high safety, while the adverse events including glare, halo and other adverse visual circumstances should be considered after triple-focus intraocular lens implantation.

ABSTRACT

A chiral high-performance liquid chromatography method was developed for the simultaneous determination of ibuprofen enantiomers in dog plasma. It was used to study the pharmacokinetics in the Beagle dog after intravenous administration of racemic-ibuprofen, S-ibuprofen and R-ibuprofen. Ketoprofen was chosen as the internal standard. After a simple precipitation using methanol as the precipitating solvent, both analytes and IS were separated on a Kromasil 100-5CHI-TBB chiral column (250 mm x4.6 mm, 5 μm) with isocratic elution using acetonitrile - 20 mmol x L(-1) phosphate buffer (pH 3.0, containing 5% methanol) (6 : 4) as the mobile phase. The detection wavelength was 220 nm. Liner calibration curves for both of the ibuprofen enantiomers were over the concentration range from 0.5 to 50 μg x mL(-1) with a lower limit of quantification of 0.5 μg x mL(-1), the accuracies were all in standard ranges. The intra- and inter- assay precisions were all below 7%. The recovery rate was 93.1% to 100.4%. The experiments proved that the method was simple, rapid and sensitive. It can be used in the quantitative determination of ibuprofen enantiomers in dog plasma. The method was used to determine the concentration of ibuprofen enantiomers in Beagle dog plasma after a single intravenous administration of racemic-ibuprofen, S-ibuprofen and R-ibuprofen (9 mg x kg(-1)) and the pharmacokinetics parameters were calculated based on the concentration-time curves. The C(max) of S-ibuprofen in Beagle dog plasma after a single intravenous administration of racemic-ibuprofen, S-ibuprofen and R-ibuprofen were 30.8 ± 4.7, 46.1 ± 5.9 and 20.0 ± 2.6 μg x mL(-1), respectively. In terms of the exposure of active ingredient, it revealed a significant difference between the administration of S-ibuprofen and the other two groups. The systematical R- to S- chiral inversion was discussed. Comparing the pharmacokinetic parameters at different doses, chiral inversion were 70.1% ± 36.6% and 76.4% ± 36.2%, respectively, after intravenous administration of racemic- and R-ibuprofen. This study provides a theoretical basis for the safety of ibuprofen formula of injection drug.

ABSTRACT

The fragmentation pathways of five estrogens (estradiol, estrone, equilin sulfate, 17 a-dihydroequilin sulfate and equilenin sulfate) have been studied with high resolution and high mass accuracy using electrospray ionization quadrupole time-of-flight mass spectrometry (ESI-Q-TOF/MS) in the negative ion mode. Molecular weights were obtained from [M-H](-) ions in the product ion spectra. The results indicate that the five structurally similar estrogens have similar fragmentation pathways. Using their stable isotope forms as internal reference compounds, the accurate mass and composition of the fragment ions were determined. During collision-induced dissociation (CID), cleavage is initiated by loss of oxygen atoms from carbon-17, after which D and C rings cleave sequentially and rearrange to finally form stable conjugate structures with highly abundant characteristic fragment ions at m/z 183 (accompanied by m/z 181), m/z 169 and m/z 145 (accompanied by m/z 143). Understanding these characteristic fragmentation pathways of estrogens will be helpful in identifying the structures of steroid hormones in general.

ABSTRACT

Objective: To break the seed dormancy and improve the germination rate of the Nirtaria sibirica seeds. Methods: N. sibirica seeds were treated with soaking in warm water, storing with moisture sand, and immersing seed in H2SO4 (98%) and gibberellic acid (GA), then the germination rate, germination vigor, and germination peak period of the seeds were determined. Results: The germination rate and germination vigor were 30% and 10% under soaking in warm water. The seeds began to germinate on the day 7 and reached the germination peak on the day 10.The germination rate and germination vigor were 55.5% and 26.5% under storing with moisture sand for 37 d. The seeds began to germinate on the day 2 and reached the germination peak on the day 10.The germination rate and germination vigor were 86.8% and 60.3% under storing with moisture sand for 95 d. The seeds began to germinate on the day 2 and reached the germination peak on the day 7.The germination rate and germination vigor were both 90.0% under being treated with 98% H2SO4 for 2 h. The seeds began to germinate on the day 1 and reached the germination peak on the day 4.Seeds after being treated with 98% H2SO4 for 2 h were sowed on the dark medium of MS + 0.5 g/mL BA + 0.5 g/mL GA, and the germination rate and germination vigor both reached to 98.1%. The seeds began to germinate on the day 1 and reached the germination peak on the day 4. Conclusion: The dormancy of N. sibirica seeds is caused by hard seed vessels. The best way of breaking the seed dormancy is first treated with 98% H2SO4 for 2 h and then cultured in the dark medium of MS + 0.5 g/mL BA + 0.5 g/mL GA, which could effectively break the dormancy of hard seeds and reach a high germination rate.

ABSTRACT

The lack of authentic standards has limited the quantitative analysis of herbal drugs in biological samples. The present work demonstrated a practicable strategy for the assay of herbs and their metabolites independent of authentic standards. A liquid chromatography- electrospray ionization-mass spectrometry [LC-ESI-MS] method for the qualitative and quantitative determination of the metabolites after oral administration of Rhizome coptidis and Zuojinwan preparation in rat urine has been developed. Urine samples, extracted with a protein precipitation procedure were separated on a C[18] column using a mixture of water [containing 0.1% formic acid] and acetonitrile [30:70, v/v] as mobile phase. The detection was performed via MS with electrospray ionization interface in positive selected reaction monitoring [SRM] mode. One urine sample after administration was selected as ‹standard›. The method validation was carried out according to a conventional method which was calibrated by authentic standards. The fully validated method was applied to the pharmacokinetic study of 2,9-demethyljateorhizine-3-sulfate, 13-methoxyjateorhizine-3- glucoronide and 6-methyljateorhizine-5-glucoronide in rat urine. The results could provide evidence to explain the combination of Rhizome coptidis and Evodiae fructus in terms of elimination

ABSTRACT

A selective and sensitive liquid chromatography-tandem mass Spectrometry [LC-MS/ MS] method was developed for the determination of berberine, palmatine and Jatrorrhizine in rat plasma. Target compounds, together with the internal standard [metronidazole], were extracted from rat plasma samples by protein precipitation with acetonitrile-methanol [1:2, v/v]. Chromatography was carried out using a C[1g] column [150 x 4.6mm, 5microm] under isocratic elution with water [containing 0.3% formic acid]-acetonitrile [30:70, v/v]. The mass spectrometric detection was performed by selected reaction monitoring [SRM] mode via electrospray ionization [ESI] source operating in positive ionization mode. The method was linear over the concentration range of 0.2-100 microg/mL for all components. The intra- and inter-day precision values were less than 14.7% and the deviations were within +/- 9.0%. The validated method was applied to the comparative pharmacokinetic studies of berberine, palmatine and Jatrorrhizine after oral administration ofRhizoma coptidis and Zuojinwan. The results indicated that the pharmacokinetics of berberine, palmatine and Jatrorrhizine were significantly different between different groups

ABSTRACT

<p><b>OBJECTIVE</b>By the detection of hepatocellular carcinoma in patients with GP73 content to explore its significance for liver cancer diagnosis and clinical treatment.</p><p><b>METHODS</b>Up-converting phosphor immunochromatography methods was used for detection of serum GP73 in 302 cases of liver cancer, chronic hepatitis, cirrhosis patients and normal control, application electrochemiluminescence detection of AFP as a comparison. And ROC curve analysis of its diagnostic performance, and patients with HCC before and after treatment GP73 periodic testing of the treatment effect.</p><p><b>RESULTS</b>GP73 content of liver cancer patients was (160.83 +/- 37.24) ng/ml, significantly higher than the chronic hepatitis, cirrhosis patients and normal controls. The area under ROC curve for GP73 diagonosis of hepatocellular carcinoma and chronic hepatitis and cirrhosis was 0. 907 which higher than the area under the ROC curve of AFP was 0. 727. Determine 110 ng/ml most of its critical value, the sensitivity was 84.7%, specificity was 75.8%; liver cancer patients with AFP < 400 ng/ml, detection rate of GP73 was 71.15%; the GP73 content after treatment showing an overall downward trend.</p><p><b>CONCLUSION</b>GP73 has a high sensitivity for the diagnosis of HCC can add to the AFP's lack of sensitivity that can be used to monitor liver cancer treatment.</p>
